Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques like Hoyer lifts. Monitor and observe patients' health status by checking vital signs, noting changes in condition, and reporting findings to healthcare professionals promptly. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote health and well-being. Support patients with medication administration following care plans while maintaining compliance with HIPAA regulations to ensure privacy and confidentiality. Provide companionship and engage clients in meaningful activities to support mental health, especially for those with dementia or Alzheimer's disease, including memory care strategies. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning living areas and assisting with home organization. Support transportation needs by providing personal transportation assistance for appointments or community outings, ensuring safety at all times. Requirements Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, dementia care, Alzheimer's care, and hospice care practices. Ability to perform patient transfers safely using equipment like Hoyer lifts and demonstrate manual handling skills. Certification in CPR and First Aid is required; additional certifications such as HIPAA training or medication administration are a plus. Experience working with individuals with disabilities, neurodevelopmental disorders, autism spectrum disorder, or developmental disabilities is advantageous. Strong communication skills to effectively interact with clients, families, and healthcare teams while respecting residents' rights. Physical ability to perform heavy lifting when necessary and work on your feet throughout the shift.
