Job Description
Job Overview We are seeking a compassionate and dedicated Home Provider to deliver exceptional care and support to individuals in a home or community setting. This vital role involves assisting residents with daily activities, ensuring their safety, and fostering a warm, respectful environment. As a Home Provider, you will play a key part in enhancing the quality of life for those you serve by providing personalized care that meets their unique needs. Duties Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor residents' health status by taking vital signs and observing for any changes in condition Administer medication following prescribed care plans and HIPAA regulations to ensure patient safety and confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Support residents with memory care, dementia, Alzheimer's, and neurodevelopmental disorders through specialized techniques and patience Operate patient transfer equipment such as Hoyer lifts and manual handling devices safely and effectively Maintain detailed documentation of care provided, resident progress, and incident reports in compliance with healthcare standards Provide personal transportation assistance for appointments or community outings while ensuring safety at all times Follow infection control protocols including cleaning, sanitation, and proper handling of medical supplies Collaborate with healthcare teams to update care plans, report concerns, and ensure comprehensive resident support Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments Strong knowledge of patient monitoring techniques including vital signs, medication administration, and patient personal care assistance Familiarity with assisted living facilities, nursing homes, home health services, hospice care, and long-term care settings Ability to perform CPR and First Aid confidently; certification preferred but not required upon hire Experience working with residents who have Alzheimer's disease or dementia requiring specialized memory care strategies Excellent communication skills to effectively interact with clients, families, healthcare professionals, and team members Physical stamina for heavy lifting, manual handling, patient transfer methods such as Hoyer lifts, and assisting with ADLs Knowledge of HIPAA regulations to ensure confidentiality of patient information at all times Ability to follow detailed care plans accurately while demonstrating empathy and patience in all interactions Join us as a Home Provider if you're passionate about making a positive impact on lives through compassionate caregiving.
