Job Summary Join our compassionate team as an In-Home Care Provider, where your dedication and expertise will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will deliver high-quality home healthcare assistance, supporting clients with daily activities, health monitoring, and emotional well-being. Your proactive approach and caring nature will foster independence and comfort for those in need, creating a safe and nurturing environment within their homes. This position offers the opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support to clients in their homes Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and transferring using patient transfer methods and equipment like Hoyer lifts Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly Administer medications accurately following medical documentation protocols and home healthcare regulations Maintain detailed medical records using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and confidentiality standards Support clients with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable living environment Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met Requirements Proven experience working with individuals with developmental disabilities, geriatrics, dementia or Alzheimer's care preferred Knowledge of medical terminology, medication administration, catheter care, and end-of-life care practices Certification in First Aid and Basic Life Support (CPR), with ongoing training in home health care assistance and hospice care if applicable Ability to lift heavy objects safely and perform patient transfers using proper techniques and equipment such as Hoyer lifts Familiarity with EMR/EHR systems for accurate documentation of patient information Strong communication skills to collaborate effectively with clients, families, and healthcare teams Valid driver's license and reliable transportation preferred for client visits within the community Compliance with home health agency standards and home & community care experience is highly valued
