Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient observations, vital signs, and overall health status to ensure well-being. Administer medications and perform catheter care following medical protocols and documentation standards. Support patients with memory care needs, including dementia and Alzheimer's disease, ensuring safety and engagement. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate medical documentation and record keeping. Follow home healthcare regulations and maintain patient confidentiality in accordance with HIPAA standards. Operate patient transfer methods such as Hoyer lifts or other assistive devices safely and effectively. Provide end-of-life care with compassion, respecting patient dignity and supporting families as needed. Assist with meal preparation, light cleaning, laundry, and maintaining a clean living environment for clients. Collaborate with healthcare professionals to coordinate care plans and communicate client needs effectively. Skills Fluency in at least two languages to facilitate effective communication with diverse clients. Strong patient observation skills coupled with experience working with individuals with developmental disabilities or geriatrics. Knowledge of medical terminology, medication administration, vital signs measurement, and basic life support (CPR/First Aid). Experience working in assisted living facilities, nursing homes, hospice care, or home healthcare agencies. Familiarity with home health care assistance, client communication strategies, and caregiving best practices. Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or patient transfer devices. Excellent organizational skills for managing medical records, documentation, and compliance with home health regulations. Compassionate caregiving attitude with the ability to handle sensitive situations professionally. Knowledge of infection control procedures including cleaning routines and laundry management. Experience working with individuals with disabilities or dementia requires patience, empathy, and adaptability.
