Role:

Join the compassionate team at Z & B Healthcare Inc / ReliantCare Hospice as a Hospice Aide in the Grand Prairie, TX area! In this meaningful role, you'll provide hands-on personal care and comfort to patients and families during life's most important moments. If you're passionate about making a difference and delivering dignified, heartfelt care, we want to hear from you. We are currently hiring Hospice Aides for Anna, TX, Dallas, TX and Fort Worth, TX.

Responsibilities:

Provide personal care services to hospice patients including bathing, grooming, and hygiene assistance Assist patients with mobility, transfers, and positioning to ensure comfort and safety Monitor and report changes in patient condition to the supervising nurse or care team Support activities of daily living (ADLs) in accordance with the patient's individualized care plan Offer compassionate emotional support to patients and their families Maintain accurate documentation of care provided during each patient visit Collaborate with the interdisciplinary hospice team to ensure quality end-of-life care

Requirements:

Current Certified Nurse Aide (CNA) certification in Texas Prior experience in hospice, home health, or long-term care preferred Strong understanding of personal care techniques and patient safety protocols Compassionate, patient, and empathetic demeanor with a commitment to dignified care Reliable transportation, valid driver's license and current care insurance to travel within the service areas Ability to work independently and as part of a collaborative care team Strong communication and documentation skills Mandatory 9 a.m. morning call to discuss patient status and care Complete documentation within 48 hours

• pay is dependent on the completion of documentation Computer knowledge a must to work within EHR system. Background check conducted upon hire. Interviews will be held via Microsoft Teams. Paper application to be completed in the Grand Prairie, TX office. Rate of pay is Per Visit, and position does not include mileage.
