Responsibilities Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using manual handling and patient transfer methods like Hoyer lifts when necessary. Monitor patients' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote healthy living. Support clients with medication administration following HIPAA guidelines and care plans established by healthcare providers. Provide companionship and emotional support while engaging clients in meaningful activities to foster mental stimulation and social interaction. Maintain a clean and safe environment through regular cleaning tasks and ensuring adherence to safety protocols. Assist with transportation for appointments or community outings, ensuring personal transportation assistance is provided safely and efficiently. Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders such as autism, or in senior care settings like assisted living or nursing homes. Strong patient observation skills with the ability to monitor vital signs and recognize signs of distress or deterioration. Knowledge of dementia care, Alzheimer's care, memory care strategies, and hospice care practices. Familiarity with care plans, medication administration procedures, HIPAA compliance, and resident's rights. Ability to perform manual handling tasks including heavy lifting and patient transfers using equipment such as Hoyer lifts safely. Excellent communication skills for effective client interaction and documentation of patient information accurately. First aid certification along with CPR training to respond swiftly in emergencies. Experience working with individuals with developmental disabilities or intellectual disabilities support is highly valued. Competence in home & community care environments with a focus on long-term care practices across diverse populations.
