Job Summary The Individualized Skills & Socialization (ISS) Service Provider supports individuals with intellectual and developmental disabilities by helping them build daily living, social, and communication skills in a structured and supportive environment. This role focuses on promoting independence, encouraging positive social interactions, and facilitating meaningful participation in community and group activities. The ISS Service Provider follows individualized service plans, implements goal-oriented activities, and ensures the safety, dignity, and well-being of each client while fostering an inclusive and engaging atmosphere. Responsibilities Provide direct support to individuals with intellectual disabilities in developing daily living, communication, and social skills based on individualized service plans. Facilitate structured activities that promote social interaction, independence, and community engagement. Assist clients with building appropriate peer relationships and practicing positive behavior strategies Collect and document data related to behaviors, progress, medication administration, and service delivery to ensure accurate records for ongoing care planning. Support clients during community outings, ensuring safety while encouraging participation and inclusion. Provide transportation and driving support for community outings or recreational activities while maintaining safety protocols. Collaborate with team members, families, and supervisors to ensure consistent care and goal alignment. Administer medications following prescribed guidelines and document administration accurately in compliance with regulations. Collaborate with families, social workers, educators, and healthcare professionals to develop and implement individualized care plans that meet each person's unique needs. Maintain a clean, organized, and supportive environment conducive to learning and development. Ensure compliance with agency policies, state regulations, and confidentiality standards. Assist clients with personal care needs, including toileting, in a respectful and dignified manner. Requirements High School Diploma or GED required. Valid driver's license Ability to pass a background check and meet state clearance requirements. Experience working with individuals with intellectual or developmental disabilities preferred. Strong communication, patience, and interpersonal skills. Must be dependable, organized, and able to work independently and as part of a team. Ability to manage behaviors using positive reinforcement and de-escalation techniques Ability to perform data collection accurately and maintain detailed documentation.
