Job Description

Job Overview Join our dedicated team as an In-Home Care Provider and make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. This vital role involves delivering compassionate, personalized care in clients' homes, promoting independence, safety, and well-being. As an In-Home Care Provider, you will be a trusted companion and caregiver, ensuring that each person receives respectful, attentive support tailored to their unique needs. This position offers a rewarding opportunity to foster positive relationships while providing essential services that enhance quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and reporting any concerns promptly Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations Prepare nutritious meals and assist with meal planning to support dietary needs Use specialized equipment such as Hoyer lifts or wheelchairs to ensure safe patient transfers and mobility assistance Maintain detailed documentation of care provided, patient observations, and progress notes in accordance with agency protocols Support clients in engaging with their community through social activities and companionship to promote mental well-being Requirements Proven experience in home & community care or assisted living environments; group home experience is a plus Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and hospice care practices Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred Ability to lift heavy objects safely and perform physically demanding tasks involving patient transfers or mobility assistance Strong communication skills to effectively collaborate with healthcare teams, families, and clients Experience working with individuals with developmental disabilities or autism is highly desirable Valid driver's license and reliable transportation may be necessary for client visits Join us if you're passionate about providing compassionate, comprehensive care that empowers individuals to live safely and comfortably at home.
