Job Description
Overview:
We are seeking a compassionate and experienced In Home Caregiver to provide high-quality care to our patients in their own homes. As a vital member of our care team, the successful candidate will be responsible for delivering patient-centered care, supporting daily living activities, and promoting independence and well-being.Responsibilities:
- Provide assistance with daily living tasks, including bathing, dressing, grooming, and toileting
- Prepare meals and snacks according to patient dietary needs
- Assist with medication management, including administration and documentation
- Support mobility and ambulation, including transferring patients
- Maintain a safe and clean living environment
- Monitor vital signs and report any changes to the medical team
- Provide emotional support and companionship to patients and their families
- Perform light housekeeping and laundry duties
- Assist with mobility and transportation to and from medical appointments
- Collaborate with healthcare professionals to develop and implement patient care plans
- Document patient care and progress
Experience:
- Previous experience as a caregiver, nurse, or healthcare professional
- Familiarity with geriatric care and hospital medicine
- Experience working in a clinic, teaching hospital, or Level II Trauma Center
- Knowledge of OR and emergency medicine procedures
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Commitment to providing high-quality patient care
