Job Description

Overview:
We are seeking a compassionate and experienced In Home Caregiver to provide high-quality care to our patients in their own homes. As a vital member of our care team, the successful candidate will be responsible for delivering patient-centered care, supporting daily living activities, and promoting independence and well-being.
Responsibilities:
  • Provide assistance with daily living tasks, including bathing, dressing, grooming, and toileting
  • Prepare meals and snacks according to patient dietary needs
  • Assist with medication management, including administration and documentation
  • Support mobility and ambulation, including transferring patients
  • Maintain a safe and clean living environment
  • Monitor vital signs and report any changes to the medical team
  • Provide emotional support and companionship to patients and their families
  • Perform light housekeeping and laundry duties
  • Assist with mobility and transportation to and from medical appointments
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Document patient care and progress
Experience:
  • Previous experience as a caregiver, nurse, or healthcare professional
  • Familiarity with geriatric care and hospital medicine
  • Experience working in a clinic, teaching hospital, or Level II Trauma Center
  • Knowledge of OR and emergency medicine procedures
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Commitment to providing high-quality patient care
