Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns promptly. Support medication administration following proper protocols while adhering to HIPAA regulations to protect patient privacy. Prepare nutritious meals and assist with meal planning to meet individual dietary needs and preferences. Use patient transfer methods such as Hoyer lifts and manual handling techniques safely to prevent injury during mobility assistance. Maintain detailed care plans tailored to each resident's specific needs, including dementia care, Alzheimer's care, memory care, and support for neurodevelopmental or intellectual disabilities. Perform light cleaning duties within residents' living areas to promote a safe and hygienic environment. Provide transportation assistance for appointments or outings using personal transportation methods when necessary. Document all interactions accurately in client communication logs and ensure compliance with home health or hospice care standards. Engage in patient observation by monitoring changes in behavior or condition and communicating updates effectively to the healthcare team. Requirements Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Certification as a CNA or equivalent caregiving credential; CPR and First Aid certification required or willing to obtain upon hire. Strong knowledge of patient monitoring techniques, medication administration procedures, and patient personal care assistance. Familiarity with HIPAA regulations to ensure confidentiality of all client information. Ability to perform heavy lifting and manual handling safely when assisting with transfers or mobility tasks. Experience working with individuals with autism, developmental disabilities, intellectual disabilities, dementia, or neurodevelopmental disorders is advantageous. Excellent communication skills for effective client interaction and teamwork within multidisciplinary settings. Ability to handle stressful situations with compassion while maintaining professionalism at all times.
