In this vital role, you will provide comprehensive support to clients in their own homes or community settings, promoting independence, safety, and well-being.

Schedule:

Mon-Fri 8a-11a, Mon and Thu 2p-5p Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patients' health by observing vital signs, medication administration, and overall condition, reporting changes promptly to healthcare professionals. Support clients with mobility and transfers using manual handling techniques and equipment such as Hoyer lifts or patient transfer methods to promote safety. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to support health goals. Provide transportation assistance for appointments, errands, or community activities, ensuring safe and reliable client mobility. Maintain detailed care plans and documentation in compliance with HIPAA regulations, ensuring confidentiality and accuracy of patient records. Engage in patient observation and communication to assess emotional well-being, especially when caring for individuals with Alzheimer's disease, dementia, autism, or other neurodevelopmental disorders.

Qualifications Must Have :

Experience with caring for Type 1

Diabetics Must Have:

Experience with caring for individuals with dementia or related disorders Proven experience working with individuals with seniors in settings such as group homes, assisted living facilities, nursing homes, or home health environments. Knowledge of patient care techniques including medication administration, vital signs monitoring, and Diabetic Care Familiarity with specialized care such as Alzheimer's care, memory care, hospice care, or dementia support is highly desirable. Ability to perform heavy lifting and manual handling safely while assisting with transfers or mobility needs. Strong communication skills for effective client interaction and collaboration with healthcare teams. Experience working with individuals with autism or neurodevelopmental disorders is a plus. Valid driver's license and reliable transportation for client outings or appointments preferred.

Pay:

$21.00 per hour Expected hours: 21.0 per week
