Sunrise Services is seeking a Caregiver to join the 16-bed Enhanced Services Facility! This is a transitional facility supporting residents with a wide range of complex needs including physical disabilities, cognitive impairment, and or mental health support needs. Duties & Responsibilities:

Support and assistance is provided in areas including, but not limited to, the following: A. Health and Safety B. Power and Choice C. Positive recognition by self and others D. Integration into the physical and social life of the community E. Developing relationships F. Promoting competence and self-reliance G. Maintaining records : Complete, as directed the following resident and personnel documentation: Medical or mental health symptoms / complaints Daily case notes Incident Reports Personal property Staff logs Time Sheets Absence Reports / Leave Requests Employee Injury Reports H. CARE assessme nts, Person Centered Service Plans and addendums, Behavioral Support Plan and other related documents if applicable: Be able to describe the purpose of and have full and complete knowledge of the contents of the above plans. Follow these plans exactly as written. Assist and support residents as directed in CARE assessments, PCSPs, Behavioral Support Plan, and Safety Plans. Give timely prompt feedback to the PCSP Team on issues related to these plans. I. Transportation In some cases may drive residents via agency owned or personal vehicle to / from medical / therapy appointments, grocery shopping or community activities.

Wage:

$20.77 - $22.03/hour All wages depend on applicable experience, license, and education.

Available Shifts:

Saturday 3pm-11pm Saturday 11pm-7am Sunday 7am-3pm Sunday 3pm-11pm Sunday 11pm-7am Friday 3pm-11pm

Requirements:

Must have High school diploma or GED and be at least 18 years of age. Prefer at least one year of experience working with people with mental / behavioral health / chemical dependency / criminal justice system issues / or experience working in similar industries. Valid WA state driver's license Acceptable 3-year driving record Car insurance with the minimum amount of $50,000 of insurance liability. DSHS background clearance required Home Care Aid Certification or Certified Nursing Assistant Certification and various other educational classes are required following hiring and at different intervals during employment. Therapeutic Options" training is required and may be obtained post hire. First aid and CPR certification is required and may be obtained post hire. Ability to lift approximately 20-30 pounds and physically assist unsteady residents Two step TB skin test is required unless proof of two step TB skin test is provided then one step TB skin test would be required unless exempt.
