Job Description
Overview Join our dedicated team as a Personal Care Worker and make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. In this vital role, you will provide compassionate assistance to residents in various care settings, ensuring their comfort, safety, and well-being. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods like Hoyer lifts. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately to nursing staff or supervisors. Support medication administration following care plans and ensuring compliance with HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist residents with meal consumption when necessary. Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols. Provide emotional support and companionship, fostering positive communication and respecting residents' dignity. Assist with transportation for appointments or outings using personal transportation assistance or mobility aids as needed. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong patient observation skills to detect changes in condition or behavior, especially in individuals with dementia or neurodevelopmental disorders such as autism. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and long-term care best practices. Ability to perform patient monitoring including vital signs measurement and patient personal care assistance with compassion. Familiarity with care plans, medication administration procedures, CPR/First Aid certification, and HIPAA compliance. Experience working with individuals with disabilities or developmental delays, supporting intellectual disabilities or senior care needs. Skilled in manual handling techniques like heavy lifting and patient transfer methods such as Hoyer lifts safely and effectively. Excellent client communication skills to ensure clear understanding of needs and preferences while providing personalized support. Ability to work independently or as part of a team in home & community care settings while maintaining high standards of safety and professionalism.
