We are seeking an empathetic, warm-hearted Community Support Worker to provide exceptional care to cognitively disabled adults residing in their own apartments in the city of Green Bay and Depere. The CCW job entails being active and present with your clients, duties include but not limited to:

• Assist clients with hygiene and personal care, including brushing teeth, bathing, using the restroom and dressing which is specific to each individual.
• Assist in administering medications and monitoring well-being, reporting changes in condition to office staff.
• Maintain a clean and organized space by completing tasks such as changing linens, washing laundry and performing basic household cleaning.
• Help individuals make menu plans and shopping lists and provide dependable assistance in completing errands, such as grocery shopping.
• Encourage physical activity, mental alertness and social involvement by planning and participating in activities such as going for walks or playing games and participating in community events.
• Provide transportation and accompany to appointments.

Also provide transportation assistance with shopping, banking, laundry, and vocational and day service needs. there isa company van on site for transportation needs This position is 40 plus hours weekly. Schedule varies during the day with hours between 8 am and 7 pm Monday through Saturday. this could be one full time or two part-time positions.

Required:

• High school diploma or equivalent degree
• Valid Driver's License
• Compassionate, trustworthy and dependable
• Ability to communicate with and listen to clients for accurate needs assessment
• Acceptable background check as required by the State of Wisconsin.

Pay:

$15.00 - $15.25 per hour
