You will deliver personalized assistance, ensuring safety, comfort, and independence for clients with diverse needs, including seniors, individuals with disabilities, and those requiring specialized memory or dementia care. Your proactive approach and empathetic nature will foster a trusting relationship with clients and their families, promoting well-being and quality of life. Responsibilities Provide direct personal care assistance, including help with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts when necessary. Monitor patients' vital signs and observe health status changes to report promptly to healthcare professionals or family members. Assist with medication administration following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Support clients in maintaining their independence through companionship, engaging activities, and community outings using personal transportation assistance if needed. Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe environment to promote overall well-being. Document care provided accurately in compliance with healthcare standards and communicate effectively with clients, families, and healthcare teams about progress or concerns. Qualifications Prior experience working with individuals with developmental disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes is highly desirable. Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), hospice care, and resident's rights. Certification in CPR and First Aid is required; additional certifications like Hoyer lift training or medication administration are a plus. Ability to perform manual handling tasks including heavy lifting safely while maintaining proper body mechanics. Strong communication skills for client interaction and documentation; experience working within HIPAA guidelines is preferred. Flexibility to work various shifts including mornings, evenings, weekends, or overnight hours to meet client needs. Valid driver's license and reliable transportation are preferred for assisting with personal transportation needs.
