About the Position Moe's Transitional Living Center, Inc. is seeking compassionate, dependable, and professional
AFH/CBRF
Caregivers to join our team supporting adults with intellectual and developmental disabilities in a residential home setting. The Caregiver provides direct care, supervision, daily living support, behavioral support, medication administration, meal preparation, and household assistance while helping residents maintain dignity, independence, safety, and quality of life. The ideal candidate is patient, dependable, attentive to detail, comfortable working independently, and committed to treating every resident with respect. Key ResponsibilitiesResident Care & Support Provide compassionate, respectful, person-centered care to residents. Become familiar with each resident's care plan, service notes, behavior support plan, medical needs, and communication log . Assist residents with activities of daily living (ADLs), including bathing, hygiene, grooming, toileting, dressing, and mobility as needed. Ensure residents are appropriately dressed and maintain proper personal hygiene. Provide supervision, redirection, and behavioral support according to individualized service plans and approved interventions. Spend meaningful one-on-one time with residents to assess their well-being, needs, concerns, and emotional state. Encourage participation in recreational activities, exercise, life-skills development, and community activities. Maintain professional boundaries and respectful communication with residents, families, guardians, case managers, and coworkers. Medication & Health Support Administer medications as assigned according to physician orders, MAR procedures, and applicable state requirements. Accurately document medication administration immediately. Document medication refusals, missed medications, and other medication-related concerns as required. Assist residents with medication reminders, appointments, and health-related follow-up as directed. Observe and promptly report changes in a resident's health, mood, mobility, appetite, cognition, or behavior. Complete required incident documentation for injuries, falls, behavioral incidents, medication errors, elopement/AWOL concerns, and other safety-related incidents. Communication & Documentation Communicate with the staff member being relieved regarding resident status, behaviors, incidents, appointments, medications, and safety concerns. Complete accurate shift communication logs and documentation. Document resident behaviors, mood, activities, participation, meals, hygiene assistance, medication refusals, incidents, and significant interactions. Maintain resident confidentiality and protect resident privacy in accordance with HIPAA and organizational policy. Communicate significant concerns to management promptly. Meals & Nutrition Prepare meals and snacks according to dietary requirements, physician orders, and resident preferences. Monitor residents who prepare their own meals as appropriate. Maintain safe food storage practices. Monitor food expiration dates and remove expired food. Maintain a clean and sanitary kitchen, dining area, and refrigerator. Wash dishes and sanitize kitchen surfaces after use. Household & Facility Responsibilities Caregivers are also responsible for maintaining a clean, safe, and organized residential environment, including: Washing, drying, folding, labeling, and distributing resident clothing and linens. Cleaning and sanitizing bathrooms. Vacuuming, sweeping, and mopping floors. Cleaning common living areas and high-traffic areas. Maintaining clean bedrooms and resident personal spaces. Monitoring household, food, hygiene, PPE, and cleaning-supply inventory. Reporting low or missing supplies to management. Performing light outdoor duties as assigned, including garbage placement and walkway safety checks. Assisting with snow removal when applicable. Securing doors and windows and ensuring the home is safe at the end of the shift. Resident Activities & Community Inclusion Ensure residents are prepared for scheduled appointments, employment, day programs, and community activities. Assist with transportation coordination and approved community outings. Support residents in meaningful community integration opportunities. Encourage residents to develop independence and practical life skills. Support residents with age

Daytime Responsibilities When residents are home during daytime hours, additional responsibilities may include: Preparing lunch and cleaning the kitchen afterward. Planning and facilitating structured activities. Documenting resident participation and engagement. Monitoring resident bedrooms and personal spaces. Supporting laundry and household responsibilities. Assisting with approved community outings and transportation. Helping residents complete appropriate household chores and life-skills activities. Training & Compliance Employees are required to participate in organizational and regulatory training, including as applicable: Medication Administration First Aid/CPR Fire Safety and Emergency Evacuation Resident Rights Abuse and Neglect Reporting Behavioral Support and De-escalation Confidentiality and HIPAA Emergency Preparedness Ongoing continuing education and required in-service training Employees must follow all applicable state regulations, organizational policies, individualized resident plans, medication procedures, safety procedures, and documentation requirements. Qualifications High school diploma or equivalent preferred. Previous experience working with adults with intellectual or developmental disabilities preferred. Previous AFH, CBRF, group home, residential care, caregiving, or direct-support experience preferred. Experience with medication administration is preferred; training may be provided as required. Must be dependable, responsible, and able to work independently. Must demonstrate strong communication and documentation skills. Must be able to maintain professional boundaries and confidentiality. Must be able to respond appropriately to behavioral and emergency situations. Must successfully complete required background checks and employment screening. Must be willing to complete all required training. Physical & Job Requirements The position may require: Standing and walking for extended periods. Assisting residents with mobility and daily living activities. Bending, lifting, reaching, and performing routine household tasks. Responding appropriately to behavioral or medical emergencies.
