This position offers an opportunity to work closely with individuals requiring assistance with daily activities, medication management, and behavioral support, fostering independence and well-being. We provide services for all adults for any age, race, or gender that is classified by the state of Wisconsin with one or more of the following needs: Advanced Aged (AA) Irreversible Dementia and Alzheimer's Developmentally Disabled (DD) Emotionally Disturbed/Mental Illness (MH) Physically Disabled (PD) Terminally Ill (TI) Traumatic Brain Injury (TBI) Diabetes Management. Depending on the resident's medical, emotional, or physical needs, staff may be required to complete additional training in the areas of the residents' unique health condition. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Administer medications accurately following prescribed protocols and document all medication activities using Electronic Medical Record (EMR) systems Monitor vital signs such as blood pressure, pulse, and temperature to ensure residents' health stability Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Maintain a clean, safe living environment by performing routine cleaning and sanitation tasks in accordance with health standards Support residents' social engagement by facilitating activities that promote socialization and emotional well-being Operate mobility aids such as Hoyer lifts safely and assist with transfers to prevent injury Document resident progress, incidents, and care activities thoroughly to ensure continuity of care Requirements Training provided previous experience preferred. Ability to work flexible shifts including evenings, weekends, or holidays as needed to support resident needs Strong communication skills to collaborate effectively with team members, residents, and families Physical ability to lift residents safely using Hoyer lifts or other mobility aids Join us in creating a nurturing environment where every resident feels valued and supported.
