Job Description
Familiar with state and federal mandates of the Medicaid Waiver, Community Care, Veteran's, Lighthouse, FAIR, Case Management and Respite Programs. Responsible for following mandates of these programs in the delivery of services. Responsible for managing all facets of the in-home care programs. Responsible for completion or delegation of all necessary forms in each of the In-home Care Programs. Monitors and counsels PCP's in the delivery of services; Work closely with Case Manager, R.N.'s and Operations Specialist; Make visits and phone contacts to clients as mandated; Monitor all paperwork to ensure compliance; Rotation of weekend call; Any other duties requested by the Director. Must be a registered nurse with a current license in WV; must have minimum of 1 year experience working with disabled populations; familiarity with people and geography of Lewis County desirable; possess excellent organization skills and supervisory experience preferred.
