Discharge planning is the process of transitioning a patient from one level of care to the next. Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. Discharge planner reports off to the Case Management team and staff.Reports to:

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Essential Functions:
Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care Keeps track of all Medicare forms and consent forms Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions Assisting with group activitiesEEOC Statement:
