Grammar Experience Full Job Description Education:
4 year degree in human service field, social services, RN, Education Full Time position Retirement Sick leave Professional case management/ Support Coordinator position. Working with a caseload of approximately 30 persons providing services to assist person with intellectual disability supports and needs. Coordination of services to those enrolled in the Alabama Medicaid Waiver services (person must meet intellectual disability criteria.) Professional position that involves assessing, planning, coordination, and monitoring AL Medicaid wavier services provided to adults and children with a diagnosis of intellectual disability. Support Coordinator completes a Person-Centered Plan for each person in their caseload and monitors to ensure Outcomes, services and supports are in place and continue to meet the needs of the person. Documentation must be current and records must be kept up to date. Quarterly bonus is available based on job performance. Training is provided for this position. Skills required - Familiar with Microsoft excel, word and additional programs utilized for documentation purposes. Use of Computer is required daily. Time Management skills Teamwork Good Grammar Background check is required Drug Test is required. Travel is required for meetings and monitoring.
