Job Description
Selected candidate will provide Case Management Services to seriously mentally ill and/or substance using adults who are currently, or have been previously, hospitalized and/or in the jail systems. Selected candidates will complete and implement case management plans with the goal of preventing future incarcerations and/or hospitalizations through linkage with community resources and continuing mental health/substance use care. Two years previous experience working in a Social Services related field is preferred. Position requires a Bachelors degree in a Social Science field. Must have a valid driver license and clean driving record.
