Job Description

Case Manager - Inpatient Mental Health Treatment Center Haven West Memphis is seeking a compassionate and experienced Case Manager. We are seeking a compassionate, organized, and motivated professional with behavioral health case management experience to help coordinate care and support patients throughout their treatment journey. Candidates must have prior experience working in a behavioral health treatment setting to be considered.
Schedule:
Monday - Friday or Tuesday-Saturday, 9:00am - 5:00pm Responsibilities Coordinate patient care from admission through discharge planning. Develop and maintain individualized treatment and discharge plans with the multidisciplinary team. Connect patients with community resources, housing, outpatient providers, and other support services. Facilitate referrals and coordinate aftercare appointments to ensure continuity of care. Collaborate with therapists, nursing, medical providers, and families to support treatment goals. Maintain timely, accurate, and compliant documentation in the electronic medical record. Advocate for patients while ensuring compliance with regulatory and accreditation standards. Qualifications Prior Case Management experience in behavioral health is strongly preferred. Behavioral health treatment experience is required (inpatient, residential, psychiatric, or substance use treatment settings). Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred. Strong knowledge of discharge planning, community resources, and care coordination. Excellent communication, organizational, and problem-solving skills. Ability to work effectively within a multidisciplinary treatment team. Why Join Us?
