Job Description

Next Steps for Families is seeking a dependable, compassionate, highly organized Case Manager to join our licensed fiduciary office. Our office provides fiduciary and case management services to vulnerable adults and protected persons who require ongoing monitoring, advocacy, coordination, and oversight. We work closely with clients, families, healthcare providers, care facilities, attorneys, courts, court-appointed professionals, and other community partners. This position requires a high level of integrity, professionalism, sound judgment, discretion, flexibility, and attention to detail . The Case Manager plays an active role in coordinating client care, monitoring client needs, communicating with providers, attending appointments, completing follow-up tasks, and ensuring that all case activity is thoroughly documented. Responsibilities Duties include, but are not limited to: Manage and monitor assigned client cases under the direction of the licensed fiduciary. Schedule and coordinate medical, dental, psychiatric, behavioral health, and other appointments for clients. Maintain and update calendars for client appointments, staff schedules, and case-related activities. Accompany clients to appointments and advocate for their needs. Prepare questions and relevant case information prior to appointments. Obtain recommendations and follow-up instructions from healthcare providers. Coordinate needs following appointments, including prescriptions, transportation, durable medical equipment, personal care items, referrals, and follow-up appointments. Request, obtain, review, organize, and maintain medical records and other client documentation. Maintain accurate and organized client files, including medical documentation and updated client photographs when needed. Follow up on tasks identified during staffing meetings and ensure assigned items are completed within appropriate timeframes. Monitor changes in clients' medical, behavioral, residential, and overall well-being and communicate significant concerns to the appropriate team members. Communicate professionally with physicians, hospitals, behavioral health providers, home health agencies, caregivers, assisted living communities, skilled nursing facilities, psychiatric facilities, attorneys, court-appointed professionals, and other community providers. Document all client-related contacts, appointments, observations, decisions, and follow-up activities accurately and promptly. Assist with coordinating client placements, services, transportation, personal needs, and other case-related matters. Conduct client visits in residential settings, care facilities, hospitals, and other community locations. Assist with inventories of a protected person's belongings and property, including homes, vehicles, furniture, personal property, cremains, and other assets. Photograph, document, organize, and assist with the preparation of inventory-related reports. Identify outstanding needs and proactively follow up to ensure appropriate services and supports are in place. Provide administrative support as needed, including answering phones, scanning, copying, filing, organizing documents, and assisting with office responsibilities. Maintain frequent communication with the fiduciary and other staff regarding client needs, schedules, concerns, and changing priorities. Perform additional case management duties as assigned. Decision-Making & Documentation Case Managers are expected to use sound judgment, identify concerns, communicate recommendations, and take initiative in addressing routine case-management needs. However, significant decisions regarding a client's finances, placement, medical care, legal matters, or other fiduciary responsibilities must be appropriately authorized by the licensed fiduciary. All actions taken on behalf of clients must be appropriately communicated and thoroughly documented. Qualifications The ideal candidate will have: Strong organizational and case-management skills. Excellent written and verbal communication skills. Exceptional attention to detail and accuracy. Strong follow-through and the ability to manage multiple active cases and competing priorities. Professionalism and discretion when working with confidential and sensitive information. Good judgment and problem-solving skills. The ability to communicate effectively and respectfully with older adults, individuals with disabilities, families, healthcare professionals, attorneys, and community partners. The ability to recognize changes or concerns regarding a client's condition and communicate them appropriately. Comfort working independently in the community while remaining connected with the office and team. Strong computer, email, calendar, documentation, and records-management skills. Reliable transportation and the ability to use a personal vehicle when a company vehicle is unavailable. A valid driver's license and reliable attendance. Flexibility with an ever-changing daily schedule. Preferred Experience Experience or education in one or more of the following areas is strongly preferred: Social work Case management Healthcare Behavioral health Senior services Disability services Caregiving Guardianship or fiduciary services Legal services Human services or a related field A bachelor's degree in social work, psychology, human services, healthcare, or a related field is preferred, but relevant professional experience may be considered. About the Position This is a hands-on case-management role for someone who enjoys meaningful work, variety, problem-solving, advocacy, and working directly with people. No two days are exactly alike. Client needs and priorities can change quickly, so flexibility and adaptability are essential . As a small, collaborative office, every team member plays an important role. We are looking for someone who is dependable, proactive, compassionate, detail-oriented, and comfortable taking ownership of their responsibilities while working closely with the licensed fiduciary and the rest of the team.
Pay:
$21.00 - $24.00 per hour Expected hours: 40.0 per week
