We are currently looking for an experienced compassionate and organized Case Manager Assistant to join our dedicated care team at Rio Vista Post-Acute in Peoria, AZ. Under the direction of the Case Manager, the case manager assistant will support our Case Management team with the following functions, but not limited to: Coordinating discharge planning and ensure proper documentation. Communicate with residents and their families to ensure they feel supported. Handling documentation and supporting tasks. Assist Case Manager with care plans and meetings. Maintain accurate patient records and case notes in PCC. Arranging safe transportation, home health services and durable medical equipment for discharging residents. Assist with referrals to other healthcare providers or community resources. Obtain the necessary documents to initiate and complete the placement process.

Qualifications:

The ideal candidate must have prior experience in healthcare, social services, or case management. Strong organizational and interpersonal communication skills. Familiarity with EMR systems and insurance processes is a plus.
