High Needs Case Manager:
JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services. Responsibilities Provides case management services to children enrolled in the behavioral-health system. Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc. Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community. Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers. Coordinates and facilitates Child & Family Team (CFT) Meetings. Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services. Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner. Qualifications Meets one of the following education/experience requirements: High School Diploma/GED required and 4 years of Behavioral Health experience. A.A. Degree and two years of Behavioral Health experience Bachelor's Degree in Social Work, Psychology, or human services field.
