Full-Time Responsibilities:

Conduct intake orientation with all patients, explaining rules, policies and procedures Complete Case Management assessments and reports Develop working relationships with outside agencies Evaluate patient needs and providing referrals as appropriate. Liaison between referral sources and provide updates on patient progress in treatment Assist patient in obtaining necessary resources (housing, food stamps, medical and benefit assistance)
