Alex, who faced addiction and homelessness himself, feared that he had survived war only to die on the streets of Oakland. After stabilizing his life, he began his mission with a few beds in a small Oakland shelter. Since then, Operation Dignity has expanded significantly, now managing eight (8) properties that provide shelter and housing to the unhoused of Alameda County. Position Summary TThe Case Manager is based at Dignity Village, an interim housing program in Alameda, CA. The Case Manager will provide comprehensive case management counseling and support to residents during their stay. This includes leveraging residents' strengths while addressing a range of issues, including substance abuse challenges, mental health conditions, general health concerns, poverty, and transitions from incarceration, street life, and chronic unhoused. The goal of the Case Manager is to assist residents in acquiring the skills and motivation needed to secure and sustain suitable housing options. Major Job Responsibilities and Duties Client Engagement & Relationship Building Establish professional, trauma-informed relationships with residents within 24 hours of intake Utilize motivational interviewing and strengths-based approaches Maintain consistent on-site presence to foster trust and community stability Deliver culturally responsive and equitable services Case Management Services Provide consistent, compassionate case management counseling Support residents in addressing behavioral health, substance use, and social stability challenges Promote client accountability, autonomy, and informed decision-making Assessment & Individualized Planning Conduct an intake and assessment within 24 hours of intake Develop an Individual Service Plan (ISP) within 48 hours with defined goals and action Identify risk factors, acuity, and service needs to inform care planning Maintain dynamic and updated SSPs aligned with client progress Housing-Focused Case Management Partner with the onsite Housing Navigator Prioritize all services toward permanent housing placement and stabilization Support access to Coordinated Entry System (CES) and housing pathways Assist clients with obtaining documentation, income, and benefits Address housing barriers such as credit, legal issues, and rental history Develop transition plans to support long-term housing retention Service Coordination & Systems Navigation Coordinate services with internal and external partners to ensure comprehensive care Facilitate connections to: Mental health and substance use treatment Medical and primary care Legal and entitlement services Employment and training programs Housing resources Track referrals and ensure follow-through Crisis Intervention, Mediation & Behavioral Support Provide crisis intervention and de-escalation to maintain safety and program stability Support conflict mediation and community cohesion Develop individualized strategies for high-acuity clients Coordinate safety planning with program leadership Progress Monitoring & Accountability Conduct regular case management sessions and check-ins Monitor participation in services, groups, and program activities Reinforce compliance with program rules and expectations Address barriers to progress and escalate concerns as needed Housing Preparation Assist clients daily in preparing for transition to housing Support development of independent living and life skills Documentation & Data Management Maintain accurate, timely, and complete client records Enter all case notes and service activities into HMIS within 24 hours Ensure compliance with funding, regulatory, and audit standards Participate in quality assurance and internal monitoring processes Incident Management & Reporting Complete and follow up on incident reports in a timely manner Ensure proper documentation and resolution Identify patterns, risks, and areas for program improvement Community Engagement & Program Participation Facilitate or support house meetings, workshops, and group activities Promote a structured, respectful, and inclusive community environment Encourage resident engagement and participation Perform other duties as assigned in support of program operations Physical Demand Ability to lift up to 50 pounds Ability to frequently use stairs Ability to stand, walk, and sit for extended periods Reasonable accommodations available Reasonable accommodation may be available for individuals with disabilities to perform the essential functions of the job.
