The Case Manager II is responsible for all the functions of the Level I Case Manager, but must demonstrate advanced proficiency in external data systems and a proven ability to handle complex, high-stakes cases in high-pressure environments. This position requires demonstrated experience in the effective coordination of high-need clients and services and the successful completion of permanent housing plans. We are looking for a person who has a can-do attitude, is passionate about engaging chronically unhoused individuals and has experience with this population. This position is located at the ECHO Shelter, Atascadero site. We are looking for someone who can work (Days and time per week), with flexibility for nights and weekends. We may consider hiring a Case Manager I position at a pay range of $22.00- 28.
Data Systems Proficiency:
Demonstrate advanced proficiency and proven experience utilizing the CenCal portal, the HMIS system, and the Coordinated Entry Program (CEP), ensuring data accuracy for reporting and grant purposes.Complex Caseload Management:
Assume a caseload that includes clients with highly complex service needs or multiple barriers to housing, utilizing advanced de-escalation skills and a trauma-informed care approach.Crisis Management:
Lead the case management response during client crisis situations, acting with calm and composure, and taking immediate, appropriate steps to mitigate health and safety risks. Communicates with clients to de-escalate situations when needed. Annual performance outcomes related to housing placements and employment goals will be established at the beginning of each year and evaluated at year's end to ensure each Case Manager is meeting expected performance standards. Consistent and reliable attendance is essential in this role to ensure clients receive dependable, uninterrupted support and continuity of care. Driving to meetings and events is a required duty for this position. Other duties as assigned. Knowledge, Skills, Talents, & Abilities Requirments Provide respectful engagement to adults with histories of chronic homelessness, serious psychiatric disabilities, and co-occurring substance use disorders. Provide culturally competent support to a diverse population. Use de-escalation skills & trauma informed care approach when issues arise. Excellent oral and written communication skills and the ability to interact with all departments/levels of the organization and outside third parties in a highly professional manner. Ability to work with diverse populations including those facing challenges in the areas of mental health, domestic violence, child welfare services, aging, trauma informed care, and low barrier shelters. Adaptability is essential in this role, as the dynamic and often unpredictable nature of a homeless shelter environment requires flexibility, sound judgment, and the ability to respond effectively to changing client and operational needs. The ability to work proactively, take initiative, and complete tasks independently with minimal supervision is essential to maintaining efficient operations and ensuring clients receive timely, consistent support. Excellent time-management and organizational skills with the ability to work independently and take initiative. Calm demeanor; Able to work under pressure; Able to respond to a crisis situation, health or safety issue and take appropriate and immediate action. Able to maintain a high-level privacy and confidentiality of employees', guests' and clients' information. Knowledge of universal precautions and what constitutes a medical or situational emergency. Understanding of mandated reporting laws. Must be professional, flexible, friendly, and work as a team player. Assume ownership for accomplishing assignments, requests and tasks. Intermediate knowledge of Office Suite, including WORD, EXCEL, and PowerPoint.Working Under Pressure:
Critical ability to work effectively under pressure and maintain a calm demeanor, taking immediate action in crisis or high-demand operational situations.Advanced Technical Experience:
Advanced experience in data entry, reporting, and navigating external agency platforms such as CenCal and HMIS.Problem Solving:
Demonstrated ability to resolve complex client issues involving multiple agencies and systemic challenges.De-escalation Skills:
Mastery in the use of de-escalation skills and a trauma-informed care approach.Attitude:
Demonstrate a professional and flexible demeanor, and the ability to work as a team player. Bilingual Spanish speaking is a plus. $0.50 bilingual pay differential is provided. Certificates, Licenses, Registrations Must possess a valid California Driver's License. Must have a reliable vehicle, clean driving record, and proof of insurance. Minimum of two years' experience as a Case Manager.
