Job Description

Responsibilities:
1.Under the direct supervision of the Supervising Care Manager (SCM), the CMA II may assume responsibility for tracking, monitoring, and review of monthly assignments and other administrative and Case management duties that promote thorough operations of the MSSP sites including: a. Monitoring Intakes from Waitlist to enrollment b. Monitoring Waitlist for changes in care and provide referrals / resources as needed 2. Coordinating client counts 3. Investigate and provide follow up for clients on Ineligible list to assist with restoring Medi-cal status 4. Managing Intakes from Waitlist to enrollment 5. Organizing all aspects of new enrollment binders and packets 6. Provide support system for SCM and CM staff to complete, review, monitor and track monthly assignments. 7. Assist SCM with reviewing completed assignments and tracking monthly assignment logs for completion a. Tracking and monitoring Monthly Progress notes for completion and timely review b. Tracking and monitoring Initial Packets for completion and timely review 8. Complete monthly phone calls and QHV to cover CM's open position (vacancies) or any uncovered CM area as assigned by SCM 9. Set up, arrange and monitor client services and document accordingly. 10. Make appropriate referrals to community resources 11. The CMA does not conduct reassessments or sign-off on the care plans.
Minimum Requirements:
A Bachelor's degree required in human services, social work or related Social Services field; and two years of direct service experience with case management services or office management skills. Direct experience with the elderly and knowledge of community resource service delivery for the elderly. Partners in Care Foundation is an equal opportunity employer. Pay:
$20.00 - $22.00 per hour
