Your Role and Responsibilities Primary responsibilities include: Individual rehabilitation services based on behavioral goals set by therapist Provide targeted case management to locate resources and services for client/family Assist families with implementation of behavioral modification techniques/concepts and psycho educational information Clinical documentation in an electronic health record system Work as part of a collaborative and multi-disciplinary team Depending on client and program needs, services are provided in person at various locations such as the office, clients homes and other field based community locations.
