Overview Provide case management services to ensure that guests/clients who are engaged in services during the seasonal shelter program are able to access services during the day, and that services do not end at the end of the shelter season. Case management services are provided with the lowest barrier possible, to people with no income or income history, disabled, and people with active substance abuse or mental health issues. Duties Under the supervision of the Program Coordinator, the Case Manager provides outreach and case management in a trauma informed and culturally sensitive manner. Assess participants' physical and mental wellness, needs, preferences and abilities and use these to develop specific goals with participants. Engage participants and develop a housing plan. On-site or in the field intake Coordinate services, referrals to community-based resources, and facilitation of transportation needs. Documentation of notable interactions in HMIS per program requirements. Participate in weekly meetings as directed by Supervisor . Perform other duties as assigned. Qualifications Bachelor's degree in social services field preferred. Valid Driver's license Current vehicle insurance One year case management experience or related services preferred . Knowledge, Skills, and Abilities Knowledge of case management. Knowledge of shelter services. Skilled in interpersonal communication. Skilled in computer use. Skilled in critical thinking and problem solving to determine the best care plan for each client. Ability to manage several different cases at once. Ability to de-escalate situations accordingly. Ability to be sensitive and responsive to diversity. Ability to work independently and with diverse populations. Ability to work on a flexible schedule including occasional evenings and weekends.
