Job description : Experienced Case Manager for the Unhoused - Shelter Experience About South County Community Services South County Community Services has been a "lighthouse" agency in South Santa Clara County to unhoused and low-income families and individuals who are in crisis situations and overburdened by the financial stress of living in Silicon Valley. We pride ourselves on compassion, responsiveness and creating valuable working partnerships with other nonprofits; small number of employees that can really make an impact; large core of community volunteers. ) Bilingual English/Spanish required Bachelor's degree or equivalent in a field related to Social Services, Mental Health, or assisting vulnerable populations Ability to develop relationships and garner trust Computer knowledge and proficiency with Microsoft Office Suite and Google Docs/Sheets Commitment to the Mission and Values of South County Community Services Must have driver's license and a clean driving record Skills and Experience Possess a strong understanding and compassion for the unhoused population we serve Trained and a current user of HMIS and the ViSPDAT assessment processes (This is mandatory) Process Cultural Competency (the ability to work, communicate, and relate to individuals from diverse and vulnerable backgrounds) Thrives in a dynamic and high activity environment Excellent at diffusing conflict with the ability to maintain composure and inform staff when additional support is needed Possesses an attentive, receptive, assertive, calm, and professional approach to dealing with critical need individuals Excellent verbal and written communication skills Excellent customer service skills Responsibilities In collaboration with other team members, this individual will build rapport with individuals experiencing homelessness, assess their needs, and connect them to services that provide stability, ease the burden of homelessness, and increase individuals' quality-of-life while they await housing and may be living in the shelter. Only those qualified and experienced with HMIS and ViSPDAT will be considered. You will assess our clients' needs, connect them to community services such as benefit assistance, provide vulnerability assessments, and provide access to a variety of referral services and other important resources. The goal is to utilize as many community services as possible to assist during the process of securing long-term, stable housing. Additionally, the case manager, with the support of the team, will be responsible for completing the required documentation to enroll people in housing programs, which may include homeless verification, verification of disability, and housing subsidy applications. DUTIES Complete intake assessment, paperwork and enroll clients into HMIS and administer

assessments Draft Stability Plan for each household to plan steps necessary towards removing barriers and securing housing Assign tasks to clients and assist, when necessary, with the footwork to help clients achieve goals Connect clients to local resources that can assist with case management goals Maintain case notes and documentation of connections to service according to HMIS protocols Ensure all documents and files are up-to-date and organized Ensure that all client data and notes are input into HMIS accurately and within 48 hours of each client meeting Meet weekly with direct supervisor for case conferencing Attend all required training for staff development Adhere to strict client confidentiality
