Benefits 401(k) and 401(k) matching Health, dental, and vision insurance Life insurance Employee assistance program Paid time off Retirement plan Job Description This on-site project-based position provides high quality, client-centered intensive supportive services to chronic individuals, families and transitional age youth in permanent supportive housing through Housing for Health. Case management duties range from outreach and engagement; intake and assessment; service planning; housing and rental assistance; on-going monitoring and follow-up; assistance with benefits establishment, transportation, and legal issues; crisis management; eviction prevention; client education; housing location services; and coordination and collaboration with HFH partners. This program includes using harm reduction strategies to enable the participants to permanently retain their rental housing. Duties and Responsibilities Outreach, Engagement & Intake Accept and process referrals from DHS and HFH partners Build rapport with clients and assess eligibility for the program Obtain necessary authorizations and consents Refer ineligible individuals to appropriate services and community linkages Conduct client intakes and assist with gathering documentation Temporary Housing, Rental & Move-In Support Assist with temporary housing referrals prior to permanent housing placement Support clients in lease-up processes and documentation coordination Coordinate move-in logistics and new tenant orientation Help clients access basic necessities such as food and hygiene items Conduct DHS-approved psychosocial assessments and update service plans every three months Ensure connections to health, mental health, and substance use services Support treatment adherence by accompanying clients to appointments Provide assistance with accessing public benefits and employment resources Support budgeting, money management, and life skills development Educate clients on tenant rights and lease responsibilities Work with property managers to address issues affecting housing stability and document eviction prevention efforts Coordinate transitions for clients with changing needs or housing situations Implement new services as required by County or HFH operations Client Satisfaction Monitor satisfaction through ongoing conversations and tenant meetings Administer an annual satisfaction survey Collaboration Participate in meetings with DHS, HFH, and community partners Facilitate regular case conferencing with supervisor and team Build partnerships with local service providers to ensure wraparound support Qualifications and Requirements Bachelor's degree in Social Work or related field and 1 year of experience with homeless individuals, families, or transitional age youth; or 2 years of experience providing direct mental health or intensive case management services Experience in social services and working knowledge of case management techniques Experience with clients facing mental health challenges, chronic health conditions, and substance use disorders Understanding of crisis intervention, harm reduction, and housing retention strategies Ability to manage a caseload of 20 clients, with strong documentation and organizational skills Proficiency in Microsoft Word and Excel Valid driver's license, ability to drive agency vehicles, and current auto insurance Commitment to maintaining professional boundaries and client confidentiality per HIPAA Attendance at mandatory trainings and meetings required Physical Working Conditions Some travel required to field locations; valid driver's license and insurance needed Prolonged sitting, standing, and frequent bending or stretching Ability to lift up to 25 lbs as needed Frequent computer use, including typing and phone communication Other Conditions This job description is not intended to be an exhaustive list of duties.
