BFFY Case Manager The BFFY Case Manager provides early intervention services, strengths-based case management, and supportive services to youth and families participating in BFFY programs—including The Friendship Club (TFC), NEO, and SAFE—for youth ages 11-25 who are at risk of or experiencing emerging mental health and substance use concerns. The Case Manager supports youth in accessing basic needs, behavioral health services, education and employment resources, and community-based supports. This is a full-time, non-exempt position supervised by the SAFE Program Manager. Case Management Provide case management services to The Friendship Club, NEO, and SAFE clients not in need of housing support. Conduct intake, needs assessments, and individualized goal planning with youth and families. Support youth in accessing healthcare, behavioral health, education, employment, and community resources. Assist with Medi-Cal, CalFresh, CalWorks, FAFSA, and related applications. Provide transportation for case plan-related support as needed. Provide crisis intervention, de-escalation support, and safety planning as appropriate. Attend Child and Family Team Meetings (CFTs), IEP/504 meetings, and other applicable multidisciplinary care meetings. Maintain professional boundaries and ethical standards. Maintain accurate and timely documentation in Salesforce and other required systems. Provide mental health/social-emotional learning support in SAFE, NEO, and TFC activities as requested. Facilitate or assist with independent living skills groups and youth support activities. Participate in collaborative case management meetings, trainings, and supervision. Assist with program evaluation, quality improvement, and data collection efforts. Other Duties Attend program events, trainings, conferences, and fundraising activities as needed. Attend agency and other program meetings as requested. Complete professional development, skill-building trainings, and other required trainings as directed by the SAFE Program Manager, Associate Director, or Executive Director. Provide general office or program support when needed. Be willing to work occasional weekends and evenings. Maintain a professional appearance and communicate effectively with supervisors and team members. Always maintain HIPAA-compliant confidentiality. Perform other related duties as assigned by the SAFE Program Manager, Finance Director/HR, Associate Director, or Executive Director. Qualifications Education & Experience Bachelor's degree in social work, psychology, human services, or related field; equivalent experience working with youth and families may be considered. Experience in social services, youth development, case management, crisis intervention, or behavioral health strongly preferred. Experience working with youth impacted by trauma, adverse childhood experiences, or mental health challenges preferred. Experience working within systems of care. Proficiency in Microsoft Office programs. Bilingual Spanish speaking is a plus. Special Requirements Valid California Driver's License. Reliable transportation and proof of insurance. Ability to safely transport youth as needed. Successful background check. CPR/First Aid certification preferred.
