In this role, you'll lead group sessions, document attendance and progress, and develop individualized service plans tailored to each client's goals and challenges. You will also evaluate needs, explore support options, and track client outcomes to ensure continued growth and success. This position involves regular collaboration with supervisors and coordination with external partners such as Social Workers, GAIN representatives, Probation Officers, and other referral agencies. We're looking for someone who communicates effectively, builds rapport easily, and demonstrates empathy, patience, and professionalism when working with individuals facing complex life circumstances.
