Under the supervision of the Program Director and/or Program Coordinator, is responsible for but not limited to planning, facilitating, implementing, and coordinating mental health / case management services to adult individuals with mental illness and co-occurring disorders at Crisis Residential Treatment Program. Essential Functions ▪ Provides mental health / case management services by exploring community resources, providing linkage, and referrals to ensure continuous system of care at CRTP. ▪ Coordinates and implements day rehabilitative program activities under the supervision of program coordinator. ▪ Coordinate with treatment team and facility staff members to provide intensive, structured services at CRTP. ▪ Coordinate with treatment team and facility staff members to ensure daily contact. ▪ Coordinates services with various public and private community health, mental health, and social service agencies in treatment of client. ▪ Reports program activities and development, crises, and any other pertinent information to appropriate chain of command ▪ Transport client(s) to connect with necessary community resources ▪ Interview clients and/or family members to obtain a psychosocial, medical, psychiatric history to assess and evaluate client's level of functioning and psychiatric condition in determining the treatment modality by which goals are to be achieved. ▪ Establish and maintain a therapeutic relationship with client and family members ▪ Assist clients, families; and significant others in understanding mental illness and gaining their acceptance of and participation in the treatment recommendations. ▪ Provide daily contact with client to ensure continuous system of care. ▪ Participate, coordinate and implement cooperative arrangements and/or team conferences with various interdisciplinary team members in the development and implementation of client's treatment plan and aftercare plan. ▪ Maintain and submit billing records in a timely manner. ▪ Maintain and update client's files and documentation in accordance with DMH, agency, CDSS CCLD, and DHCS guidelines. ▪ Must drive to multiple job sites. ▪ Perform other tasks and responsibilities as assigned by the Program Director and/or Program Coordinator. Minimum Qualifications
  • Knowledge, Skills and Abilities Required ▪ Bachelor Degree in Social Work, Psychology or other related field from an accredited college.
▪ Employment eligibility required. ▪ Must have own reliable transportation, valid California Driver License, and proof of automobile insurance coverage. ▪
Abilities:
Ability to drive; Ability to develop and maintain effective working relations with program and with community representatives and contractors; Ability to work both independently and as a member of a team; ▪ Minimum Requirements Regular attendance and background check required Initial Health Screening and TB clearance required

Skills:
Good writing and oral (communication) skills are necessary; Strong interpersonal and social skills; Time management skills necessary
  • Preferred one year of direct service experience in a hospital or mental health setting which has an interdisciplinary team approach.
  • Ability to speak Spanish or one of Asian languages (Korean, Vietnamese, or Chinese preferred)
  • Overtime, holiday, or weekend work may be required.
▪ Supervisory Responsibilities None identified. ▪ Environmental Conditions (Working Conditions) Job duties are to be performed both in office and field settings. ▪ Physical Requirements The employee is expected to drive, sit, stand, walk, carry a maximum of 20lbs, listen, and speak in order to complete job requirements. ▪ Mental Requirements The employee must be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.
