Job Description

Job Summary We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will coordinate patient care plans, facilitate communication among healthcare providers, and ensure seamless discharge planning. Your expertise will help improve patient outcomes, optimize resource utilization, and support individuals from diverse backgrounds through personalized case management strategies. Duties Assess patient needs through comprehensive intake interviews and medical record reviews, utilizing EMR (Electronic Medical Record) systems and EHR (Electronic Health Record) platforms. Develop individualized care plans that incorporate medical, social, behavioral health, and community resources to promote optimal recovery and well-being. Coordinate with multidisciplinary teams—including nursing, social work, physicians, and specialists—to facilitate timely interventions and discharge planning. Conduct utilization review and management to ensure appropriate level of care while adhering to HIPAA regulations and health insurance guidelines. Provide ongoing client communication, offering education on treatment options, medication management, and available social services. Collaborate with social services and community agencies to connect patients with necessary support systems such as housing, transportation, or mental health services. Document all case management activities accurately in EMR/EHR systems, maintaining compliance with healthcare standards and confidentiality protocols. Requirements Proven experience in treatment facility/mental health is highly desirable. Strong background in social work, or related healthcare fields experience preferred. Familiarity with managed care principles, utilization review/management, discharge planning, and health insurance processes. Proficiency in Microsoft Office applications along with experience using EMR/EHR systems for documentation and communication. Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds. Ability to handle complex cases involving behavioral health Strong organizational skills with attention to detail in case documentation and follow-up activities. Pay:
$25.00 - $30.00 per hour
