Transitional Housing Reports to: Program Manager llFLSA Status:
Full-time, Non-Exempt The Transitional Housing Case Manager provides comprehensive, trauma-informed case management services to veterans participating in the Transitional Housing Program. The Case Manager supports veterans in achieving housing stability, increasing income, improving wellness, and transitioning to permanent housing. This position requires a strong understanding of veteran-specific resources, including Veteran Affairs systems of care and community-based support services. This position manages referrals through the Coordinated Entry System (CES) and walk-ins into the Transitional Housing Program.RESPONSIBILITIES AND MAJOR DUTIES
Conduct comprehensive intake assessments to identify barriers to housing stability, income, healthcare, and overall well-being Develop individualized service plans in collaboration with each veteran, incorporating short- and long-term goals Provide ongoing weekly case management through regular one-on-one meetings and progress evaluations Assist veterans in securing permanent housing and maintaining stability using a Housing First approach Connect veterans to VA benefits, healthcare services, mental health treatment, substance use services, and other veteran-specific resources Support veterans in obtaining employment, education, vocational training, and income benefits (VA compensation, SSI/SSDI, etc.
Qualifications Qualifications:
AOD Registered and/or Certified, or BA/BS in related field, preferred Basic understanding of the military and the Veterans Administration Minimum of a Social Work or related field preferred Minimum of 6 months of paid case management experience in related field, preferred. Prefer prior experience working with homeless population Knowledge of HMIS databases a plus! Ability to prioritize competing responsibilities and thrive in challenging situations Able to change priorities at a moment's notice Must be able to fluently read, write and communicate in English Must be able to comfortably, both mentally and physically, perform all aspects of job description and other duties as assigned by supervisor Veteran Status a Plus!Physical Requirements:
Ability to bend and lift light objects Ability to sit for extended periods of time while viewing a computer monitor Dexterity of hands and fingers to operate a computer keyboard Capable of hearing and speaking to exchange information Seeing to read reports and data Ability to speak English Must be able to work in a noisy, busy environment Share job details toSimilar remote jobs
