Job Description

Raphael House of San Francisco, Inc. Residential Case Manager Position Summary Our mission is to help low-income families and families experiencing homelessness strengthen family bonds by achieving stable housing and financial independence. The Residential Shelter Program provides nourishing and respectful structure to families who have experienced homelessness. We tailor services for each family to provide them with the necessary support, education, and skills to assist them in obtaining housing, employment and self-sufficiency. Families work directly with caseworkers and residential program staff to address individual barriers to housing in order to develop a long-term plan for success. Under the supervision of the Director of Family Services, the Bilingual Residential Case Manager is responsible for working with the multidisciplinary team to develop a comprehensive action plan for each family that includes housing, financial literacy, wellness services, education and child development support, and workforce development skill-building leading toward employability.
Key Responsibilities:
Be thoroughly knowledgeable of the mission and operating principles of Raphael House. Maintain full client caseload and actively work toward meeting annual goals. Conduct eligibility assessments and Informational Interviews with prospective clients. Conduct intakes, assess needs, and review program expectations and case plan agreements with families. Maintain consistent one-on-one case management meetings on a weekly basis and as needed to provide emotional support and coaching in concert with addressing case management needs. Case Manager will also be available for crisis intervention. Fairly and consistently uphold all program policies and procedures. Facilitate action-oriented collaboration with the parent(s). Work with each family to develop and implement mutually agreed upon techniques to address both concrete and underlying challenges, such as emotional and/or behavioral challenges that may impede family stability. Work with each family to assist them in becoming housed, employed, and self-sufficient. Assist with removing barriers to stable housing--support with addressing mental health needs, childcare, maintaining sobriety, monthly budgeting and saving, etc. Assist families in accessing services, attending housing appointments, etc. (local travel is required). Assist parent(s) in maintaining goal-oriented objectives and establishing reasonable expectations toward the family goal of departure from the Residential Program and entrance into affordable housing. Reinforce forward thinking, helping to establish timelines for departure. Respect that each family functions uniquely and independently, and always work with families utilizing a strengths-based perspective. Acknowledge that challenges will exist and see those challenges as potential learning opportunities for the parent and their family to work through. Commit to establishing a strong partnership with the family in order to help the family overcome said challenges and obstacles. Proactively introduce Residential families to our Bridge Program through intake and exit date from the Residential Program. Regularly evaluate goals, plans, and strategies for each family with your supervisor and collaboratively with the Family Services team. Proactively refer clients to internal and external services. Co-facilitate a weekly residential community meeting. Establish and maintain strong relationships with community service providers. Be willing to participate in public speaking engagements on behalf of Raphael House. Establish meaningful intern and volunteer opportunities. May provide regular supervision and ongoing training opportunities to interns. Assist with coordination of community workshops around housing, health and nutrition, financial literacy, mental health and wellness, etc.
General:
Uphold the principles and standards of Raphael House. Provide excellent client-facing service by being approachable, respectful, warm, empathetic, and calm under pressure. Operate from a trauma-informed perspective, understanding that lived experiences shape us and all individuals have their own story, strengths, and challenges. Respectful communication towards all community members, including staff and clients, is expected. Value and support work within a multidisciplinary team and work cooperatively with all other departments. Establish and maintain healthy professional boundaries with families while building trusting relationships. Uphold the highest level of client confidentiality. Follow agency and state law procedures, including reporting suspected child and elder abuse. Be dependable, adaptable, receptive to change, willing to learn, and take a cooperative and creative approach to problem solving. Attend staff meetings and staff training as scheduled. Meet weekly with the Director of Family Services for supervision. Maintain up-to-date client files, case planning, case notes, statistics, and all other required data entry in the database and/or spreadsheets, as required. Other duties as assigned.
Qualifications and Skills:
Bilingual in English and Spanish preferred. Bachelor's Degree in social services, social work, psychology, child development, or related field preferred. In lieu of a Bachelor's Degree, relevant work experience will be considered. Ability to work a flexible schedule. This position is M-F, with some evenings required for client meetings and/or onsite activities. Operate with cultural humility and respect for all, and commit to working positively with staff and clients with different backgrounds and lived experiences. Experience working with individuals and families experiencing homelessness preferred. One year of well-documented case management experience is preferred. Experience working with vulnerable populations in a professional setting will be considered. Prior experience with leading groups and curriculum development preferred. Able to remain calm under pressure and utilize strong conflict management and crisis intervention skills. Must possess excellent verbal and written communication skills. A writing sample may be requested.
Essential Functions:
Able to work independently and seek supervision when appropriate. Must be able to prioritize and manage multiple tasks and deadlines. Strong time management skills are essential. Able to remain calm under pressure and utilize strong conflict management and crisis intervention skills. This position is required to work on the computer for several hours per day. Able to navigate the Windows operating system and use Word, Excel, Google Suite Applications (Gmail, Calendar, Meet, Drive, Docs, Sheets, etc.), Adobe Acrobat, etc. Prior experience with Apricot, Clarity, or other client management software is a plus.
Salary:
$71,000 to $75,000 commensurate with experience.
