Program Manager Position Overview A nonprofit organization is seeking a compassionate and committed Outreach Case Manager to support individuals experiencing homelessness. This role uses a "whatever it takes" approach to help clients stabilize, access services, and transition into permanent housing. The Case Manager will provide field-based outreach, assessments, housing navigation, crisis intervention, and ongoing case management support. Key Responsibilities Conduct intake assessments and develop individualized service plans focusing on housing, income, health needs, and personal goals. Provide referrals and linkages to healthcare, mental health, substance use treatment, benefits, and community resources. Complete program assessments such as VI-SPDAT and perform 90-day service plan reviews. Maintain accurate, timely documentation, case notes, and client files according to program standards and confidentiality requirements. Build strong, supportive relationships with clients using trauma-informed care and motivational interviewing. Provide crisis intervention, safety planning, and de-escalation as needed. Collaborate with internal teams, community partners, and coordinated entry systems to support client progress. Participate in required trainings, staff meetings, and program initiatives. Maintain knowledge of community resources to deliver wrap-around support. Perform field work in various outdoor and community settings. Preferred Qualifications Experience in homeless services, case management, social services, or working with vulnerable populations (2+ years preferred). Familiarity with crisis intervention, harm reduction, trauma-informed care, and housing navigation. General Requirements Ability to work flexible hours, including occasional evenings or weekends. Ability to obtain CPR/First Aid certification (provided). Must pass background checks and provide eligibility to work in the U.S. Valid CA driver's license, personal vehicle, and insurance required. TB test clearance.
