N/A Social Services Position Summary The Case Manager provides intensive, individualized case management to individuals and families residing in scattered-site interim housing throughout Ventura County. Participants include families with children and Transitional Age Youth (TAY), with household needs potentially spanning behavioral health, physical health, child and youth development, education, employment, income, benefits, childcare, family functioning, and other areas affecting housing stability. The Case Manager works with each household from entry with the goal of achieving a timely and successful transition to permanent housing. Through comprehensive assessment, individualized service planning, frequent engagement, resource coordination, and close collaboration with Housing Navigators and other service providers, the Case Manager helps households address immediate needs, reduce barriers to permanent housing, increase income and stability, and develop the skills and resources necessary to successfully assume the responsibilities of a permanent tenancy. The position requires a family-centered approach that recognizes the needs of the household as a whole while ensuring that the distinct needs of children, youth, TAY, parents, and other household members are appropriately identified and addressed.
