Full Job Description Position Summary:
The Case Manager provides direct support and advocacy to clients by assessing needs, developing service plans, linking families to community resources, and monitoring progress. The case manager is a grant-funded position that works primarily with our emergency shelter clients and in crisis or emergencies. This role ensures clients receive comprehensive, coordinated services that promote stability, self-sufficiency, and overall well-being.
Key Responsibilities:
Provides case services and crisis intervention to victims of domestic violence; understands managing crisis situations in a trauma informed manner. Conducts personal needs assessments and develops appropriate action plans for assigned clients and their children. Provides social, legal, medical and another advocacy needs as determined to support each assigned client Performs a variety of routine duties including follow up with clients and maintaining records of services provided and other such responsibilities as assigned. Provide ongoing case management including referrals, advocacy, and follow-up. Maintain regular contact with clients through home visits, phone calls, or office meetings. Collaborate with community partners, service providers, and agency staff to coordinate resources. Accurately document client interactions, progress, and outcomes in a timely manner. Prepare reports and maintain records in accordance with agency policies and grant requirements. Participate in team meetings, supervision, and professional development opportunities. Maintain client confidentiality and uphold ethical and professional standards at all times. Provide 24-hour hotline coverage on a rotating basis, responding to domestic violence calls as needed.
Qualifications:
Associate's or Bachelor's degree in a related field preferred; equivalent work experience may be considered. 2+ years of experience in program coordination, event planning, or project management. Must work well with persons of varied sexual preference, racial, ethnic, cultural, socioeconomic backgrounds, and with individuals who are differently-abled. Must complete a 40-hour certified domestic violence counselor training program prior to employment. Must be at least 21 years of age. Must receive clearance from the Department of Justice Criminal Record Offender Information Data Base. Excellent organizational, verbal, and written communication skills. Leadership skills to initiate change in client behavior, to facilitate and maintain effective relationships with clients and their children and to serve as a stable, non-judgmental role model for survivors and their children Ability to work both independently and collaboratively in a fast-paced environment.• Proficient in Microsoft Office Suite, Google Workspace, and other relevant software.
Work Environment & Physical Requirements:
Work is performed in a combination of office, client home, and community settings. Frequent travel within the community is required. Position requires sitting, standing, walking, and driving for extended periods. Must be able to lift, push, and pull up to 25 pounds on occasion. May require occasional bending, reaching, and repetitive hand motions (typing, writing, handling materials). Occasional evening or weekend hours may be required to meet family needs.
