Job Description

We are seeking a Case Manager II for our BRAVE Program in Willows, CA (Glenn, County), who will report to the Program Supervisor or Program Manager. The Level 2 Case Manager will provide support and services to individuals and families who are facing challenges or issues addressed by NVCSS. The Case Manager will work closely with clients to assess their needs, develop individualized service plans, and connect them with appropriate resources and support services. This individual will serve as an advocate and liaison for clients, helping them navigate complex systems and overcome barriers to success. There are three levels of case management based on program and client needs. By having structured educational requirements and defined responsibilities for each level, this approach ensures that case managers are well-prepared and appropriately qualified to meet the varying needs of their clients. The Level 2 Case Manager handles a caseload of individuals with moderate needs, requiring a more in-depth approach to case management. This role involves more complex service coordination, client advocacy, and thorough documentation to ensure effective support and intervention.
Duties/Responsibilities:
Case Manager II- Moderate Cases Conduct comprehensive assessments to determine client needs. Develop, implement, and adjust individualized service plans. Monitor client progress and outcomes through regular follow-ups. Coordinate and collaborate with multiple service providers. Provide extensive advocacy and support for clients. Maintain thorough and accurate case records and documentation. Conduct crisis intervention as necessary. Facilitate access to community resources and support services.
Education and Experience:
Bachelor of Arts (BA) degree in Social Work, Human Services, Psychology, or a related field. Minimum of 2 years of experience in case management or a related field. Strong understanding of community resources and service coordination. Excellent communication, organizational, and problem-solving skills.
Examples of Duties:
Conduct in-depth assessments to identify complex client needs. Develop and execute comprehensive, individualized service plans. Monitor and evaluate client progress and adjust plans as necessary. Coordinate with a wide range of service providers and agencies. Provide high-level advocacy and support for clients. Maintain meticulous case records and documentation. Handle crisis situations and provide immediate intervention. Facilitate access to specialized services and community resources. Mentor and provide guidance to less experienced case managers.
Education and Experience:
Bachelor of Arts (BA) degree in Social Work, Human Services, Psychology, or a related field. Minimum of 5 years of experience in case management or a related field, with a focus on complex cases. Case Manager Positions:
Strong assessment, problem-solving, and decision-making skills, with the ability to develop creative solutions to complex problems. Excellent communication and interpersonal skills, with the ability to establish rapport with clients from diverse backgrounds and build trusting relationships. Knowledge of community resources and support services available to individuals and families in specific geographical settings. Ability to work independently and as part of a team, with a collaborative and client-centered approach. Cultural competency and sensitivity to the needs and experiences of diverse populations. Proficiency in computer skills and case management software. Valid driver's license and reliable transportation, as occasional travel may be required. Commitment to the mission and values of NVCSS, with a passion for serving others and making a positive impact in the community. Ability to work in an office setting, which may include shared workspaces. Effective time management skills to balance administrative tasks, client appointments, meetings, and documentation requirements within scheduled work hours.
Physical Requirements:
Must be able to remain in a stationary position during shift. Must be able to move freely, including standing, walking, bending and reaching. Must be able to adhere to process protocol including safety. Able to drive in rural and city environments. Moves equipment weighing up to 50 pounds.

While performing the duties of this Job, the employee is regularly required to sit; use hands to handle or feel and talk or hear. The employee is frequently required to reach with hands and arms. The employee is occasionally required to stand; walk and stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Involves the potential for high stress levels and exposure to hostile situations; some non-standard work hours may be required.
