Full Time (40 hours per week) All Roads (which operates Boulder Shelter for the Homeless) has been a fixture in the community since 1982, providing a bed, meals, services and dignity to those experiencing homelessness in our community. Our mission is to create avenues to stable housing for our community's homeless adults, from a foundation of safe shelter and supportive services. All Roads has a strong culture of collaboration, communication, creativity, compassion and support, which yields an inclusive and engaging dynamic within the organization.
Job Summary:
All Roads is seeking a Case Manager to work in our Permanent Supportive Housing program. PSH provides the bridge to homeless adults entering stable housing by providing on-going intensive case management. Around 90% of the adults who enter our program never return to homelessness.
Primary Responsibilities:
Assist former chronically homeless, and/or disabled clients acquire and maintain housing, increase self-sufficiency, and achieve stability. Ensure all verbal communications, documentation, and workspace practices strictly adhere to HIPAA privacy and security standards Assist clients in obtaining and maintaining public assistance and income such as food stamps, Medicaid, SSI/SSDI, employment, vocational rehab, etc. Help clients connect with services to address all matters of physical health, mental health, substance use, and prescription needs. Write and maintain case notes that meet documentation standards for Medicaid billing, ensuring accuracy, timeliness, and compliance with HIPAA regulations. Make appropriate referrals to other community programs and agencies, as needed. Assist clients with basic needs such as transportation, clothing, ID cards, eyeglasses, dental care, utilities, and more. Assist clients with money management, budgeting, and savings plans. Assist clients in developing social support networks. Refer, screen, and perform intake duties for potential program candidates. Assist program candidates with program paperwork, voucher acquisition, apartment search, lease-up, furniture acquisition and move in. Maintain effective communication and relations with partnering landlords to mediate or resolve client/landlord conflicts. Develop and maintain effective communication and relations with voucher administrators concerning all matters of client voucher maintenance and program compliance. Maintain thorough knowledge of the social service network in Boulder County and develop excellent working relationships with providers from other agencies. Maintain client statistics and information Attend trainings as needed. Regularly conduct home visits and other duties as assigned.
Education & Experience:
Bachelor's degree, 3 years of relevant experience, or Qualified Behavioral Health Assistant Certification required [MM1] Minimum 6 months' experience working in homeless services. Clinical experience in one or more of the following areas preferred: homelessness, substance abuse, mental illness, criminal justice, poverty. B.A. in social work, psychology, or related field preferred. Valid driver's license is required for travel to clients' places of residence.

Excellent one-on-one client skills. Excellent collaborative and communication skills. Ability to establish and maintain productive relationships with clients/colleagues/community partners holding different perspectives and/or opposing needs. Excellent organizational and multi-tasking skills. Ability to address competing priorities. Demonstrated ability to work independently, be self-motivated, exhibit resiliency, maintain strong boundaries and practice excellent self-care. Must be able to move 20 pounds, remain in a stationary position for long periods of time, exchange information, and move about workspaces. Driver's license with acceptable driving history, and the ability to travel to client residences required. Compensation:
Starting wage is $30.99 per hour + $1.50 per hour bilingual pay differential, if applicable.
