Job Description
Community Connections, Inc. is hiring an additional full-time Case Manager for our Cortez office. This hybrid position supports Medicaid-eligible adults and children with disabilities in Montezuma and Dolores Counties. Flexible schedule after training, full benefits, unlimited PTO, and $25.49/hour.
Requirements:
Bachelor's degree in a relevant field; five years of relevant experience in case management, care coordination, or developmental disabilities; or an equivalent combination of education and experience.Please complete application at https:
