04/10/2026 The Long-Term Services and Supports (LTSS) Case Manager is responsible for the assessment, planning, monitoring, and referring activities for people with long term care needs including people who are elderly, blind, physically disabled or living with brain or spinal cord injuries and people with significant mental health needs. Spanish bilingual preferred with a $1.00 increase in pay rate.

Requirements:

Bachelor's degree required or five years of relevant experience in the field of LTSS Advocacy strongly preferred. Driving is a requirement. Must have a valid driver's license with an acceptable driving record

• 2 full time positions available at $27.05
• 29.75/hr. DOE/DOQ.
• Approximate accrual of 15 days per year paid time off to start, paid holidays, paid sick leave, and a full range of Medical, Dental, Vision, Life and Disability and EAP, in addition, we offer 401K and Colorado Paid Family and Medical Leave Insurance Program (FAMLI).
