Primary Role:
The Case Manager at Homeward Bound of the Grand Valley (HBGV) provides comprehensive, client-centered case management services to individuals and families experiencing homelessness. The Case Manager works directly with clients to assess their immediate needs, identify barriers to housing stability, and develop individualized, strengths-based plans of care. These plans are designed to help clients establish realistic goals, access appropriate resources, and make measurable progress toward permanent housing and greater self-sufficiency. The Case Manager connects clients with community resources and supportive services, including housing programs, employment assistance, health care, behavioral health services, transportation, identification documents, public benefits, and other essential supports. The position also assists clients with completing applications, gathering documentation, attending appointments, and navigating complex service systems. Through regular meetings, the Case Manager monitors each client's progress, updates service plans, maintains accurate and timely case records, and provides ongoing encouragement and accountability. The Case Manager collaborates with HBGV staff, housing providers, government agencies, nonprofit organizations, and other community partners to coordinate services and promote successful outcomes. This position requires compassion, professionalism, sound judgment, and the ability to establish appropriate boundaries while working with people from diverse backgrounds. The Case Manager supports HBGV's mission by helping clients overcome barriers, build stability, and move toward safe, sustainable, independent housing.
Responsibilities:
- Work directly with HBGV guests and the community to promote housing, support services, and residential life skills.
- Coordinate and interact with outside agencies on behalf of guests/clients.
- Evaluate and assist guests/clients with the development of a service plan for attaining permanent housing, health services, skills training programs, and other internal and external community support services (e.g. government-issued identification, employment, etc.).
- Meet regularly with guests/clients to ensure appropriate progress on service plan activities; provide guest/client coaching on an as-needed basis.
- Document guest/client information and accurately maintain current files.
- Ensure compliance with agencies regarding the release of information, HIPAA, and PHI.
- Facilitate the process of applying for food stamps (SNAP), Medicaid/Medicare, Disability, L.E.A.P., etc.
- Professionally and respectfully resolve conflict while maintaining composure under pressure.
- Work independently, prioritize workloads, and complete a wide variety of tasks in a timely and accurate manner.
- Deescalate aggressive behaviors without means of restraint as needed.
Requirements:
- Work experience in social work, psychology, counseling, case management experience, or equivalent work with the homeless population or in a similar environment.
- Compelled and motivated to provide appropriate assistance to homeless individuals and families.
- Excellent communication and interpersonal skills.
- Experience with Microsoft Windows, Microsoft Office, Google Workspace, etc.
- Fluent in English (Spanish is a plus)
Preferred Credentials:
- Knowledge of mental health, medical and/or electronic health record technologies, and software.
- Prior experience working with at-risk youth, biological families, adults with developmental disabilities, and other diverse populations within the community.
