We are currently seeking an IFP Case Worker to provide intensive, home-based intervention services to DCF referred families for purpose of preventing child's out of home placement, improve parenting, safety, and case management interventions along with improving positive parent - child relationships.

Open 8:00am - 7:00pm;

Hours Vary By Day & Location Expected Schedule:

Full-Time, 40 hours/week;

Flexible Outline of Duties and Responsibilities:

Provide outreach and engagement to client upon referral to the program by conducting an initial phone intake process with the family, collecting basic information and presenting information to team to determine eligibility. Complete an assessment of the family including both strengths and areas of need through home visits and various assessments to develop a care plan. Provide weekly face-to-face direct service hours to client including support, advocacy, referrals and assistance with the development of positive parenting, safety planning, basic life skills and identified goals in collaboration with the Department of Children and Families in regard to their involvement. Participate in monthly in person joint sessions with the Department of Families and the family in regard to the Service Plan established and strengths and challenges within. Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations, PIE records, EPIC system documentation and closing summaries. Why UCFS?
