Job Description

The Center for Human Development, (CHD) is seeking a SOAR (SSDI Outreach Access Recovery) Outreach Caseworker to join our Homeless Outreach program in Waterbury, CT. Your role as an
Unsheltered Outreach Caseworker:
The Unsheltered Outreach Caseworker oversees a variety of cases through different venues on a daily basis in Waterbury, CT and is able to provide the unsheltered seeking assistance the necessary tools to accomplish their goals. They will maintain a schedule including but not limited to hours of actual street outreach, use of equipment and supplies as well as a schedule of external support providers that may able to further the individuals' attempts to end their homelessness. Also will work within the program's model as well as protocols established by the Western CT Mental Health Network and the Coordinated Access Network with whom we jointly run the outreach services establishing open communication, support and accountability among collaborating agencies.
REQUIRMENTS
High school diploma or GED required Bilingual in Spanish and English Two years in a human service organization preferred Two years' experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations Individuals with lived experience encouraged to apply Bachelor's degree from an accredited educational institution preferred Familiarity with PATH program funding Reliable, registered & insured vehicle to transport clients and approved driving record check Approved criminal background check (CORI)

The Unsheltered Outreach Caseworker should actively support the philosophy of the Agency and maintain a high level of commitment to program consumers and staff.

Philosophical commitment to the concept of human potential Cheerful, out-going personality High energy level; self initiating Directive and goal oriented Assertive and confident Performs well under stress and in the event of crisis Open, direct interpersonal style; works well as a team member Demonstrates patience, caring, and compassion Ability to set limits Open, direct interpersonal style; works well as a team member and with supervisor Organized, Motivated, Dependable Flexible (i.e. adjusting hours to meet consumer & program needs, being trained in new techniques and strategies, etc.)
SCHEDULE
Monday-Friday 8:30am-4:30pm Take advantage of a competitive compensation package starting at $22.00/hr. As well as a phenomenal benefit package that includes, Dental, Health and Life insurance. Paid time off, earned vacation time and paid holidays just to name a few.
