Description:
Responsibilities include but are not limited to: Screening program applicants for program eligibility; Conducting assessments Create individualized service plans Provide individualized support to program participants including life skills training and job readiness activities Connect program clients with the appropriate resources and develop community resource network Monitor clients progress Provide ongoing support Documenting in electronic medical record Data Collection and Reporting as assigned Incumbent will focus on wraparound services, including case management, housing assistance, access to healthcare, and other essential resources, complements the existing continuum of care. It enhances the support available to individuals by addressing the various needs and challenges they may face beyond addiction treatment. This comprehensive approach helps individuals maintain recovery and provides a safety net to prevent relapse. Incumbent will actively collaborate with community organizations, such as nonprofit agencies, housing providers, and educational institutions, to enhance the continuum of care. This collaboration strengthens the network of services available to individuals in need of addiction services and fosters a community-wide approach to supporting recovery and long-term well-being.
Requirements:
Experience in case management Knowledge of wraparound services Possess strong interpersonal and communication skills Ability to present training curriculum in group format Ability to develop community resource network Computer skills required
